Anybody know what can cause a photomultiplier tube to shatter inside the vacuum chamber?

Running a PE Turbomass Gold. Was unable to achieve vacuum, rechecked all seals, cleaned inner source and replaced. Still only able to achieve very minimal vacuum. Checked roughing pump, rechecked all seals again! Finally opened vacuum chamber to find the photomultiplier in pieces!!

This is going to be an expensive part to replace!

Application of line voltage to a DC input can casue that to happen (please don't ask me how I know this).
