Just curious what do your think about the most challenging/most difficult part facing the LC/MS analysis? How do you usually deal with it? Thanks.

Depends on what you are doing...

If it is analyses from a biological matrix, I would think that ion-suppresion is the general problem.

To reduce ion suppression stemming from a complex matrix such as biofluids, you need to reduce the complexity of the sample with solid sample clean-up techniques such as SPE and high-quality chromatographic separations.

You can test for ion-suppression issues by doing post--column infusion of the analyte(s).
