Spreadsheets in Data Collection

All,
We recently had an SOP training on Data Collection and Evaluation. The question I had was how does one go about including a microsoft excel spreadsheet with calculations in a notebook? Should two spreadsheets be included, one with the formulas in the cells and one witht he actual numerical data? This I feel is slightly excessive, any thoughts?

I would put one copy of the spreadsheet with the formulas in the notebook as a reference and then include only the data pages. This assumes that there are no changes to the formulas, calculations, etc. However, this may not be stringent enough for SOP and/or GLP rules.

If the spreadsheet is locked down so that only certain cells are editable, then one cross referenced copy of the formulas should be adequate.
If spreadsheets change regularly, formula listings and a copy of the spreadsheet should be either in the notebook or filed with other materials associated with the analysis (all referenced, of course).
We stick a copy of the spreadsheet in the notebook & a copy both normal & showing formulas in a binder along with printed CDS materials, method copy, protocol copy and any other germane printouts that we don't want to glue into the notebook.
Then it all gets reviewed. That's the person who gets to double check the spreadsheet formulas, confirm some of the calculations etc.
