Your autoinjector can't properly actuate the 6 port valve.
This error is usually caused by wear over time resulting in polishing of the mated surfaces. Once they are polished enough to allow for significantly more contact than with new components, the actuating motor can no longer overcome the friction between them, causing the error.
At the very least, a qualified person needs to open up the 6 port valve and inspect the rotor and stator. It would probably be best to go ahead and replace those parts.
Thanks,
DR
